*All new patient/client appointments require a deposit of $150. This will go towards your final invoice. This deposit is forfeited if the appointment is canceled with <24 hours’ notice. We will need all new patient veterinary medical records at least 48 hours before the appointment for Dr. Blankenship to review. There is a medical record review fee of $75. New client/patient forms are available on our home page, please bring these to your appointment or email to appointment@animalsfirstvet.com if possible*
